In the “Questionnaires” tab of the RxScale Admin Tool, you’ll find an overview of all created questionnaires, including their type, status, and submission numbers – along with intuitive options to create and manage each form.
1. Questionnaire Overview – Table Structure
A table lists all questionnaires with the following columns:

Name | Internal and public title of the questionnaire |
Type | Type of questionnaire – Product Recommender or Direct To Cart |
Created At | Date of creation |
Updated At | Date of last update |
Submissions | Number of submissions (daily access possible) |
Actions | Actions like View, Delete, Duplicate |

Search Field : Live search by name
View: Opens the detailed view of the questionnaireDelete: Permanently removes the questionnaireDuplicate: Creates a copy for use as a template
2. Create a New Questionnaire
Click
+ Create NewEnter the Questionnaire Name and choose the Questionnaire Type (Product Recommender or Direct To Cart)
Click
Addto create the new form
You will then be taken directly to the detail view for further setup.
3. Detail View – Tabs and Features
After creating a questionnaire, you can manage and configure it through several sections:
General
Configure Display Name, Questionnaire ID (for embedding)
Customize appearance using Installation Snippet and Custom CSS
Editor
Click
Open Editorto access the drag-and-drop builderAdd/edit questions (e.g. multiple choice, text, scales)
Recommenders (only for Product Recommender)
Click
Add Newto define product logicSet up trigger conditions based on user responses
Link matching products or results
Doctor Mapping
Click
Add New Mappingto define which form fields are visible to the doctorConfigure: Name, Operator (e.g. CONCAT, AGE, FILE), and Value (question or page reference)
Only the selected mapped fields will be shown in the doctor’s interface.
4. Activation & Tracking
Once your questionnaire is complete, embed it into your site.
Track responses using View Daily Submissions for detailed insights into usage and completion.
